solely caused by the effects of the cosmetic facial
surgery.

In spite of the lack of benefits from treatment, she has
continued to avail herself of treatment in the hope of
finding relief. She has made every effort to improve her
conditions and there is no foundation to any implication
that she has failed to avail herself of treatment or that
there exist treatments that would have remedied her
condition and enabled her to return to work. Her
emotional and physical reactions are profound and
incapacitating and this incapacitation is ongoing. She has
been unable to resume work solely as a result of her
emotional and physical condition arising from the surgical
outcome.
